New Version of Nonimmigrant Visa Application Form (OF-156) Posted Mar 30, 2001 On March 12, 2001, the U.S. Department of State (DOS) announced that a revised form would be available from March 16, 2001 for the processing of nonimmigrant visa applications. This new version of the standard nonimmigrant visa application form known as the OF-156 contains several minor corrections. Consulates may use the prior version of the form until they run out. The new form was issued in PDF format so consulates can print it locally. The form is available through the DOS WebSite.  DOS plans to issue a completely redesigned nonimmigrant visa application form, which will be known as the DS-156, in July 2001, so the OF-156 should be used only through June 30, 2001. © The Law Office of Sheela Murthy, P.C.
