Following are links to audio instructions to help you locate these documents that are required for your AOS/485 filing.
IMPORTANT! Instructions on how to label/name your document files.
Arrest Disclosure. Each applicant MUST fill a separate form.
ALL non-blank pages of current and expired passports.
ALL approval notices (I-129, I-539, I-20s/SEVIS I-20s, IAP66s, J waivers, EADs, etc.). If the notice was originally issued with an I-94 attached, please include a copy of the corresponding I-94.
I-94 arrival / departure card from your last entry into the U.S. If you also have copies of any prior I-94 cards received since your initial entry into the U.S. please also provide copies of those.
Driver’s license or state identification card showing current address.
Copy of social security card, if applicable.
Primary birth document.
Primary marriage document.
Secondary birth evidence, if applicable: Secondary birth evidence.
Two (2) Birth Affidavits, if applicable: Birth affidavit instructions and template.
If previously married, copy of divorce / annulment decree/s or death certificate/s as proof of termination of marriage, if applicable.
Translations, if applicable: Translations.
If you have a Derivative (spouse or child) over the age of 18 who will be filing with you, please submit a Dependent Spouse Dual Consent form signed by you and the Derivative.
